Housing Supports

Four Rivers Behavioral Health offers a range of housing support options for individuals with intellectual or developmental disabilities, designed to promote stability, independence, and community living.

Housing supports vary in structure and level of assistance to meet individual needs.

Group Homes

Four Rivers Behavioral Health serves as the management agent for two HUD-subsidized group homes.

Eligibility Requirements:

  • Diagnosis of a developmental disability

  • Must meet HUD minimum income requirements

Applications for group homes may be obtained by contacting Housing Supports.

Riverwood Group Home – Paducah
  • Housing for eight individuals

  • Four men and four women

Willow Apartments Group Home – Mayfield
  • Housing for 8 individuals

  • Four men and four women

Fuller Apartments

Fuller Apartments is a HUD-subsidized independent living facility offering:

  • 12 one-bedroom apartments

  • Each unit includes a kitchen, living room, and bathroom

Eligibility Requirements:

  • Documented disability

  • Must meet minimum income requirements

  • Must be able to live independently

This option is designed for individuals seeking greater independence with minimal support.

Residential Services

Family Home & Adult Foster Care

Four Rivers Behavioral Health may place individuals with:

  • Family Home Providers (FHP) or

  • Adult Foster Care Providers (AFC)

These providers are trained by the agency, and individuals are integrated into the family unit.

Case Management & Support

Housing supports are coordinated through the Center for Integrated Supports Case Management.

Case Management helps ensure services are coordinated, appropriate, and aligned with individual goals.

Staffed Residences

Staffed residences provide a transitional living environment for individuals who:

  • Have been discharged from an institution, or

  • Need alternative housing due to changes in living arrangements

  • Housing for three individuals per residence

  • Each individual has their own private room

  • Trained staff provide:

    • Assistance with daily needs

    • Nutritious meal preparation

    • Support accessing the community and participating in chosen activities

Four Rivers Behavioral Health currently operates:

  • Two staffed residences in Graves County

  • Two staffed residences in McCracken County
