Assertive Community Treatment (ACT) is a comprehensive, community-based service designed to support individuals who have been diagnosed with a serious mental illness.
The goal of ACT is to provide coordinated, ongoing care that helps individuals live a full and meaningful life, supported in the community and not defined by their diagnosis.
ACT services are provided through the Center for Integrated Supports Case Management.
Care is delivered in the community and tailored to each individual’s needs, with services available at:

With ACT, individuals receive help with both daily living needs and long-term goals.
Support may include assistance with:
Medication management and daily routines
Identifying effective therapies and services
Coordinating care with providers and support systems
Finding housing & applying for benefits or food assistance
Returning to school and/or finding and maintaining employment
By identifying the right mix of services, the ACT team helps ensure individuals are using all available resources to support recovery and stability.
ACT is designed to be highly individualized.
ACT teams work with a relatively small group of participants, allowing services to be specifically designed to meet each person’s unique needs.
There is no time limit for participation in the ACT program. Services and supports continue for as long as they are needed.
Multiple ACT team members work with each participant on an ongoing basis. Regular contact helps ensure consistency, follow-through, and progress toward treatment goals.
ACT services are delivered by a multidisciplinary team, which may include:
Psychiatrists
Nurses
Mental health professionals
Employment specialists
Case managers
Peer support professionals
Community support staff
Substance use specialists
This team-based model ensures care is coordinated, comprehensive, and responsive.
ACT teams work with participants to find the best time and place to meet, adjusting to individual schedules whenever possible.
The goal is to make participation in ACT accessible, flexible, and supportive, reducing barriers to care.
ACT services are available 24 hours a day, 7 days a week.
Someone is always available to respond to urgent needs or emergencies. The ACT team serves as a partner in care, supporting individuals as they work toward their personal goals.
