Group Homes
Four Rivers Behavioral Health is the management agent for the HUD subsidized Riverwood Group Home, in Paducah, and the Willow Apartments Group Home, in Mayfield. Each group home provides housing for eight persons, Riverwood houses four males and four females and Willow houses eight males. Applicants must have a diagnosis of a developmental disability and meet minimum income requirements. Applications may be obtained by calling Housing Supports.
Fuller Apartments
Fuller Apartments is a 12 unit, HUD subsidized independent living facility. Each unit is a one-bedroom apartment with a kitchen, living room and bathroom. Applicants must have a documented disability, meet minimum income requirements and be able to live independently.
Residential Services
Residential services are provided by Four Rivers Behavioral Health to individuals with mental retardation who have been accepted into the Supports for Community Living (SCL) Program, by the Department of Medicaid Services.
First, Four Rivers Behavioral Health can place individuals with families who have been trained by the agency as Family Home Provider (FHP) or Adult Foster Care Providers (AFC). The individuals are integrated into the family unit. Second, the agency’s Staffed Residences provide a transitional living arrangement for individuals diagnosed with mental retardation who have been discharged from an institution, or due to other circumstances in living arrangements, must find alternative residences.
Our staffed residences are managed by trained staff and provide housing for three individuals. This creates a family environment. Each person is provided with their own room. Residential staff provides assistance with daily needs, prepare nutritious meals and help each person access the community and participate in activities of their choice. We currently operate two staffed residences in Graves County and two staffed residences in McCracken County.
