What is Assertive Community Treatment?

Assertive Community Treatment is a way to deliver a full range of services to people who have been diagnosed with a serious mental illness. The goal of Assertive Community Treatment is to give program participants adequate community care and help them live a full life that is not defined by their mental illness. With ACT, consumers get help with their basic needs – like taking medications, getting up in the morning, and getting through the day. The ACT team also works closely with our clients as well as others in their care team. The ACT staff member working with the participant may help review which medications work best for the consumer, what therapies may be a good fit and more. By identifying services that may be beneficial, the ACT team can ensure that client’s are getting the best possible care and utilizing all the resources available to them. The type of help that can be offered through the ACT Program is broad reaching. Program participants can get assistance in finding housing, applying for food stamps, going back to school, getting a job and more.

How Does it Work?

Services are provided where they are needed most:

Clients receive ACT services in their homes, where they work, or other settings in the community where problems can occur or support is needed.

Personalized Care:

The ACT team works with a relative small group of program participants. This means that each person is given care specifically designed to meet their needs.

Unlimited Support:

Our team will help secure selected services and supports based on the individual needs of the consumer for as long as they need them. There is no time limit for how long someone can be in the program.

Continuous Care:

Several ACT team members work regularly with each consumer. Being in regular contact with program participants ensures details aren’t missed and that the client is always moving forward in their treatment plan.

Our Team Approach:

Psychiatrist, nurses, mental health professionals, employment specialists, case managers, peer support professionals, community support staff, and substance abuse specialists join together to give consumers ongoing, individuals care.

Flexible Care:

We work with program participants to find the best time and place to meet. We work around client’s schedules to ensure they do not miss an appointment and that working with the ACT team is as easy as possible.

Comprehensive Care:

Our teams provide an array of services to help meet our client’s needs. It could be as simple as helping someone find a job they love or more involved by managing a hosts of available services that could be of benefit to the client.

ACT services are available 24 hours a day, 7 days a week. Someone is always available to handle emergencies. We want to be your partner in helping you achieve your individual goals.
