Welcome to the MyRecord – Consumer Portal
Four Rivers Behavioral Health (FRBH) is pleased to offer you access to your behavioral health records through our Consumer Portal. After registering for the Consumer Portal, consumers will have the ability to view different parts of their record, send secure messages to their provider, and even request medication refills. Here is a list of some of the information you will be able to view will be:
- Name and contact information
- Current medication list and allergies
- Demographic information
- Your schedule
- Summaries of your office visits
Consumers are able to sign up for the Consumer Portal when they arrive for their appointment at the check-in window, or by mailing a completed registration form to their providers office. You can access the registration form here.
If you have questions about any of the content in your behavioral health record or if you need assistance, you are encouraged to contact your FRBH Service Provider.
